It was upsetting because he would gag on the secretions from his trach, which is likely colonized. The increased respiratory effort of clearing the secretions triggers the low-flow alarm resulting from the increased intrathoracic pressure.


We have had a few teenage fentanyl intoxications post-ROSC with circulatory collapse requiring VA-ECMO. These young people devastate their families, and they usually require prohibitively expensive inpatient rehab. I cannot imagine having a child accidentally or deliberately doing this. The parents are angry and blame the staff.

My mother, brother and I took his two children for a walk and then to dinner. We like walking in this particular cemetery in the early evening when the deer lie down between the boxwood hedges and headstones. There are many older growth oaks here. There is little risk of motorists running over pedestrians.

I always think that if we jumped into the routines of small children part of you, perhaps ten percent, would reflect on your former happiness without being tied down into routines. "Wow, I underappreciated eating at a decent restaurant without a howling child."
