I thought about you prior to reading the The Children of Violence tetrology. I should revisit Martha Quest.
Complicated UTI with pyelonephritis and calculi; after the nephrostomy tube was placed to relieve the acute hydronephrosis, she became tachycardia, hyperthermic (106 rectal)...septic, from a highly perfused colony of ESBL Gram negative bacteria with lipopolysaccharide endotoxins promoting vascular collapse? Invanz/ertapenem, a carbapenem, treats complicated UTIs as a last resort, given its liver toxicity. What did we learn today? Pretreat with abx prior to interventions? With a fever and symptomatic? Yes.
Carbapenems like imipenem were synthesizeds by Merck from Streptomyces cattelya-derived thienamycin (1976).
