I had a patient with this book on his trayside table(!) I asked him if he had a subscription to the nyrb or read the online review. His daughter is an architect in Amsterdam, and his son-in-law gave him this book. He had a valve replacement, but there was no explanation in the procedural note about the blood products and protamine administered afterward.

He stood up later that night, with a burst of loculated old blood from a pericardial window, and around the bulb drain clogged there. The dressing to be taken down and the drain stripped with no further problems.
