I do not have any intrinsic motivation to study for the CMC exam. It is not obvious to me what that would accomplish.

One of the pages in the CMC book regarded the anticoagulant argatroban; dispensed in 50 mL glass vials, and usually running at a kg conversion of 10-15 mL/h. Someone was telling me about autoimmune heparin-induced thrombocytopenia in a patient who died with mottled legs from this condition, which is very painful, almost mimicking sickening cell crisis-like vaso-occlusion.

This is the nurse who had a patient weaned from a dobutamine drip on the precipice of cardiogenic shock, low urine output, and in shock. After back and forth with the provider, the thought was to give IV fluids in the setting of his chronically low ventricular ejection fraction and low output state so he could go to the nursing home Monday morning. His blood pressure and UO improved when I left Sunday night.

He coded Monday with a sitter in place for exiting and confusion in a diabetic coma with a blood sugar < 20 mg/dL. The shock state depletes glucose, and the night team did not check his blood sugar or offer him oral intake. The nurse that told me about the heparin coded him; they used a Lund Cardiopulmonary Assist System (LUCAS) that battered his chest wall to a pulp, and he was resuscitated for 40 minutes unsuccessfully. I did not work Wednesday night because the charge Monday morning changed the assignment as a parting shot for what she thought I left for her Monday morning.

One can argue whether it is my fault, but the consensus is that it was. As I get older, and more morally erect in posture, I have fewer excuses.

I saw Solaneum virginianum, yellow-fruited nightshade, all over the farm I visited. None of the animals would eat it; so it remained, without the application of broadleaf herbicide, glyphosate.

I failed the CMC within one point. My lowest score in the breakdown was in monitoring and diagnostics, and this requires that I review the murmurs and timing in the cardiac cycle, many of those paired with the intraaortic balloon pump.
